Although this is a membership practice, there is no contract; however, you will have to sign paperwork consenting to receive treatment.
The cost of membership is $3000 per year or $300 per month. It is your choice about which way you pay. There is a savings of $600 if you pay the entire year up front. If you are unhappy with our care or decide to go back to the insurance practice world, you can just let us know.
There is a minimum charge of 3 months.
You will not be charged until you see a provider in person.
